망막미세혈관병변과 뇌소혈관질환의 시간에 따른 진행 사이의 연관성

Abstract

Background

We investigated the mutual relationship of the progress of retinal microvascular lesions with that of cerebral small vessel diseases (CSVD) over time.

Methods

We prospectively included acute ischemic stroke patients. The presence of cerebral microbleeds (CMB), lacunar infarcts, leukoaraiosis and retinal microvascular lesions were interpreted. The retinal microvascular lesions through retinal photography included arterio-venular nicking, narrow arteriole, focal arterial narrowing, enhanced light reflex, and widened venule. Follow-up of the brain and eye evaluation was planned about one year after the initial examination. The progress of cerebral small vessel diseases and retinal microvascular lesions was assessed. Univariate analysis to test the relationship between retinal microvascular changes and CSVD changes was performed. Multiple logistic regression model was used to analyze the independent relationship between retinal microvascular changes and CSVD changes over time.

Results

Total 98 patients (63 male) were enrolled. The mean time interval between the initial and follow-up exam was 15.8 months. Initial eye exam showed arterio-venular nicking in 72/88 (81.8%), enhanced light reflex in 56/88 (63.6%), focal arterial narrowing in 71/88 (80.7%), narrow arteriole in 84/88 (95.5%), and widened venule in 78/88 (88.6%). Progression of retinal microvascular lesions could be evaluated in 55 patients. Among them, progression was observed in 22 (40%). CMB progressed in 13/66 (19.7%), lacunar infarct in 13/76 (17.1%) and leukoaraiosis in 10/76 (10.2%). The progression of retinal microvascular lesions was not correlated with the progression of any type of cerebral small vessel diseases. The presence of hypertension was the independent factor which is significantly associated with the progression of CSVD.

Conclusion

The progression of retinal microvascular lesions was not correlated with the progression of cerebral small vessel diseases.

서 론

망막미세혈관병변의 발생은 뇌백질변성, 열공성뇌경색, 미세뇌출혈과 같은 뇌소혈관질환과 비슷한 병태생리에 의한다[1]. 전향적 코호트 연구의 정상 인구집단에서 망막미세혈관병변은 뇌소혈관질환의 존재와 유의한 연관성을 보였다[2]. 다른 뇌졸중아형에 비하여 열공성 뇌경색 환자에서 망막미세혈관병증이 더 많이 관찰되었다[3]. 뇌졸중이 없었던 55세 이상의 환자에서 망막미세혈관병증은 뇌백질변성의 악화를 예측하는 인자로 나타났다[4]. 뇌미세출혈에 관하여는 한 치매 역학 연구 코호트에서 망막미세혈관병변과 뇌미세출혈의 연관성에 대하여 보고된 바가 있을 뿐이다[5].
시간에 따른 망막미세혈관병변의 변화와 뇌소혈관질환 변화의 상호 연관성에 대한 연구는 지금까지 없다. 두 병변의 변화양상이 연관되어 있는지 규명하는 것은 뇌소혈관질환의 변화를 대변하는 표지자로 망막혈관병변의 역할을 추정할 수 있게 할 것이다. 뇌소혈관질환 변화를 확인하는데 고비용의 영상검사가 필요한 상황에서 망막혈관병변을 진찰하여 이를 대치할 수 있다면 임상진료나 임상 연구를 진행하는데 있어 비용 효과적일 수 있다.
이러한 배경으로 시간에 따른 망막혈관병변의 변화가 뇌소혈관질환의 변화와 상호 연관이 있는지 알아보고자 한다.

대상 및 방법

2010년 4월부터 2013년 7월까지 증상 발생 7일 이내의 급성 뇌경색 환자가 입원하였고, 이 중 연구 참여에 동의한 환자를 전향적으로 등록하였다. 급성 뇌경색은 확산강조자기공명영상을 촬영하여 진단하였으며, 연구자 판단에 따라 뇌경색병변이 커서 뇌소혈관질환을 평가할 수 없는 환자는 제외하였다. 임상변수들로 연령, 성별, 위험인자(고혈압, 당뇨병, 고지혈증, 흡연력), 뇌졸중아형을 조사하였다. 뇌혈관검사(컴퓨터단층촬영혈관촬영술, 자기공명혈관촬영술), 심전도, 활동심전도를 시행하여 뇌졸중원인을 분석하였다.
뇌자기공명영상 기법 중, 기울기에코영상(Gradient-echo image), T2 강조영상(T2-weighted image), 액체감쇠역전회복영상(fluid attenuated inversion recovery image)을 시행하였다. 뇌소혈관질환으로 미세뇌출혈, 뇌백질변성과 열공경색을 포함하였다. 독립된 연구자가 미세뇌출혈, 열공경색(급성 뇌경색과 무관한)과 뇌백질변성을 판정하였다. 뇌백질변성은 Fazeka’s 등급으로 평가하였다[6]. 뇌백질변성은 뇌실 주변과 심부로 나누어 평가하였고, 각각 0-3점까지 점수를 매겼다. 열공경색은 관통동맥폐색에 의하여 발생한 것으로 여겨지는 3-15 mm의 국소 고신호강도가 액체감쇠역전회복영상 또는 T2강조영상에서 관찰되는 경우로 정의하였다. 또한, 열공경색을 평가할 때 현재의 뇌졸중증상의 원인이 되는 급성 뇌경색병변은 제외하였다. 미세뇌출혈은 기울기에코영상에서 보았을 때 뇌실질에 존재하고 저신호강도를 보이는 직경 10 mm 이하의 둥근 국소병변으로 정의하였다[7]. 뇌소혈관질환의 진행은 처음 촬영한 영상과 비교하였을 때, 추적 촬영한 자기공명영상에서 미세뇌출혈과 열공경색의 경우 하나 이상 증가, 백질변성의 경우 Fazekas 등급이 1 이상 변화하였을 때 정의하였다. 망막혈관촬영술과정은 1% 트로피카마이드(tropicamide) 안구점적으로 동공확대를 시킨 후 진행되었다. 양안의 안저부위에서 디지털망막영상을 촬영하였다. 환자의 임상정보를 모르는 안과의사가 망막미세혈관병변을 평가하였다. 망막미세혈관병변으로 동정맥협착(arteriovenular nicking), 좁아진 세동맥(narrowed arterioles), 국소동맥좁아짐(focal arterial narrowing), 과장된 빛반사(enhanced light reflex), 넓어진 정맥(widened venules)을 관찰하였다(Fig. 1). 그러한 변화는 과장된 빛반사의 경우, 없는 경우가 grade 0으로 하여 1, 2로 분류하였고, 동정맥 협착의 경우 grade 0-3, 국소동맥좁아짐, 좁아진 세동맥, 넓어진 정맥은 grade 0-4로 분류하였다[8,9]. 망막미세혈관병변은 안구의 어떠한 사분역에 존재하더라도 ‘있음’으로 판정하였다. 망막미세혈관병변의 전빈적인 중등도를 파악하기 위하여 각 망막혈관병변의 정도(0-4)를 모두 합하여 총망막혈관점수(total eye score, 총점=17점)라고 임의로 정의하였다. 뇌소혈관질환과 안저검사를 통한 망막혈관병변의 추적검사는 첫 검사 후 대략 1년 후에 시행하였다. 망막미세혈관병변의 진행은 ‘경미함’ 이상의 어떠한 변화도 진행으로 정의하였다. 뇌소혈관질환의 진행은 백질변성, 미세뇌출혈, 열공경색 중 하나라도 악화한 경우를 ‘악화’로 정의하였다.
본 연구는 임상연구윤리위원회의 허가를 받았으며 참가자로부터 사전동의를 모두 받았다. 인구학적 데이터와 망막미세혈관병변, 뇌소혈관질환에 대하여 기술분석을 시행하였다. 뇌소혈관질환과 망막미세혈관병변의 연관성을 규명하고 그들의 시간에 따른 변화들 역시 상호 연관성이 있는지 분석하기 위하여 Pearson 카이제곱 또는 Fisher의 정확한 검정, 로지스틱 회귀분석을 시행하였다.

결 과

1. 인구학적 특성

2010년 4월부터 2013년 7월까지 총 98명의 환자가 등록되었다. 평균 나이는 63.5세(표준편차, 11.3)였으며 63.6%가 남자였다. 가장 많은 뇌졸중 유형은 소동맥질환이었으며(42.9%), 다음으로 큰동맥질환(29.6%)과 심장성 색전증(13.3%) 순이었다(Table 1).
초기 확산강조영상, T2강조영상, 액체감쇠역전회복영상이 모든 환자에서 시행되었다. 초기 기울기에코영상은 89명에서만 시행되었다. 추적검사는 T2강조영상 또는 액체감쇠역전회복영상이 76명에서 시행되었고, 기울기에코영상은 73명에서 시행되었다. 초기 안과검진은 88명의 환자가 받았고 10명의 환자는 안저 사진의 판독이 부정확하여 평가하지 못하였다. 28명의 환자가 추적 안저검사를 받지 못하여 55명이 두 번째 안저검사를 받았다. 두 안저검사의 간격은 15.8개월(표준편차, 7.7)이었다.

2. 초기 및 추적검사상 망막미세혈관병변 및 뇌소혈관질환 소견

초기 뇌자기공명영상에서 백질변성은 84/98 (85.7%), 열공경색은 55/98 (56.1%), 미세뇌출혈은 32/89 (36.0%)에서 관찰되었고, 백질변성과 열공경색이 있는 환자의 나이가 더 많았다(Table 1). 백질변성 분석 결과, 뇌실주변은 77/98 (78.6%), 심부 백질변성은 76/98 (77.6%)에서 관찰되었다. 추적 검사에서 백질변성은 66/76 (86.8%), 열공경색은 57.9%, 미세뇌출혈은 41.7%에서 관찰되었다(Table 2).
초기 안검진 결과, 동정맥협착이 72/88 (81.8%), 과장된 빛 반사가 56/88 (63.6%), 국소동맥 좁아짐은 71/88 (80.7%), 좁아진 세동맥은 84/88 (95.5%), 넓어진 정맥은 78/88 (88.6%)에서 발견되었다. 추적 관찰한 안검진에서는 동정맥협착이 49/60 (81.7%), 과장된 빛반사가 42/60 (70%), 국소동맥 좁아짐이 50/60 (83.3%), 좁아진 세동맥이 57/60 (95%), 넓어진 정맥은 54/60 (90%)에서 관찰되었다(Table 3).

3. 뇌소혈관질환과 망막미세혈관병변의 연관성: 단면분석

초기 검사 결과를 분석하여 볼 때, 과장된 빛반사, 국소동맥 좁아짐, 넓어진 정맥이 백질변성의 유무와 유의한 연관성을 보였다(p =0.003, 0.025, and 0.049; Table 4). 열공경색은 동정맥협착, 넓어진 정맥과 유의한 연관성을 보였다(p =0.049, 0.040; Table 4). 총망막혈관점수에 있어서도 백질변성과 열공경색이 있는 환자에서 유의하게 높았다(p =0.001, 0.021; Table 4). 그러나 미세뇌출혈과 망막미세혈관병변 사이 연관성은 찾을 수 없었다(Table 4).

4. 망막미세혈관병변과 뇌소혈관질환의 시간에 따른 변화와 상호연관성

망막미세혈관병변과 뇌소혈관질환의 시간에 따른 변화의 상호연관성은 55명의 환자에서 평가할 수 있었다. 그들 중 동정맥협착은 1명에서, 과장된 빛반사가 2명, 국소세동맥협착 11명, 좁아진 세동맥 10명, 넓어진 정맥이 5명에서 악화하였다(Table 5). 로지스틱 회귀분석을 통하여 나이, 성별, 위험인자(고혈압, 당뇨병, 흡연, 고지혈증)를 보정하여 볼 때, 망막미세혈관병변의 진행과 뇌소혈관질환의 변화는 유의한 연관성이 없었으며, 고혈압만이 뇌소혈관질환의 악화와 유의한 연관성을 보였다(교차비, 16.0; 95% 신뢰구간, 1.009-253.506; 유의확률, 0.049).

고 찰

이 연구는 처음으로 망막미세혈관병변과 뇌소혈관질환의 시간에 따른 변화가 함께 일어나는지를 보고자 하였다. 망막미세혈관병변과 뇌소혈관질환의 악화는 함께 발생하지 않았다. 게다가 망막혈관병변은 뇌소혈관질환의 각 유형에 따라 연관성 유무가 차이가 있었다. 뇌소혈관질환 중 미세뇌출혈은 망막미세혈관병변의 어떠한 종류와도 연관성을 보이지 않았다.
기존 연구에서 미세뇌출혈을 제외한 뇌소혈관질환과 망막미세혈관병변의 연관성은 보고된 바 있다(Table 6). ARIC 연구에서 55세 이상의 정상 성인에서 보인 망막미세혈관병변의 진행은 시간에 따른 백질변성과 열공경색의 진행을 예측하는 인자였다[4]. 급성 허혈성 뇌경색 환자에서 열공경색과 백질변성이 망막미세혈관병변과 관련 있음은 이미 보고된 바 있다[10]. 그러나 미세뇌출혈이 망막미세혈관병변과 연관성이 있는지에 대한 연구는 부족하였다. 최근 중국 연구에서 망막정맥직경(venular caliber), 세동맥 프렉탈크기(arteriolar fractal dimensions), 세동맥 비틀림(arteriolar tortuosity)과 미세뇌혈관의 연관성이 보고되었고, 그 연구에서 오히려 백질변성과 열공공색은 망막미세혈관병변과 관련이 없어 기존의 알려진 결과와 상이한 결과를 보여주었다[5]. 본 연구에 의하면 백질변성과 열공경색은 망막미세혈관병변과 연관성이 있던 반면에 미세뇌출혈은 망막미세혈관병변과 유의한 연관성이 없었다. 망막미세혈관병변과 미세뇌출혈의 연관성에 대해서는 논란이 있지만, 본 연구진들은 뇌소혈관질환의 아형별로 다른 연관성을 보일 것으로 추정한다. 본 연구에 의하면 망막미세혈관병변은 출혈성 기전을 보이는 미세뇌출혈보다는 열공경색 및 백질변성과 같은 허혈성 기전을 가진 소혈관질환과 보다 연관이 있어 보인다[11]. 미세뇌출혈은 망막미세혈관병변의 발생과는 다른 병리기전을 가질 수 있겠다.
1년 이상의 추적 관찰을 통하여 보았을 때 망막혈관과 뇌소혈관병변의 진행은 상호 연관성이 관찰되지 않았으며 고혈압만이 뇌소혈관병변의 악화와 연관된 인자였다. 다시 말하면, 망막미세혈관병변과 뇌소혈관질환은 함께 변화하지 않았다. 등록된 환자 수가 적었고 추적 관찰 시기가 짧아서 유의한 결론을 유도하기에는 한계가 있다. 또한 등록된 환자 수가 적어서 임상변수들의 보정이 어려워, 보다 대규모의 전향적 연구가 필요하다. 두 번째 한계점으로, 본 연구에서는 심장탓뇌졸중이 적었고 소동맥질환의 비율이 높아 뇌경색 환자군을 대표한다고 보기는 어렵다. 이는 병변이 큰 뇌경색 환자들의 경우 소혈관질환 평가가 어려우면서 연구등록에서 배제된 것이 그 피할 수 없었던 원인으로 본다. 셋째, 본 연구에 등록된 대상은 정상 성인이 아닌 급성 뇌경색 환자들이었으므로 해석에 유의를 요한다. 따라서 급성 뇌경색이 큰동맥질환에 의한 경우 공존하는 소혈관질환에 대한 평가가 만성기와 차이가 있을 수 있겠다.
본 연구 결과 망막미세혈관병변의 시간에 따른 변화는 뇌소혈관질환 변화와 상호 연관성이 없었다. 또한 뇌백질변성과 열공경색과는 달리 미세뇌출혈의 존재는 망막미세혈관병변과 연관성이 없었다. 따라서, 뇌소혈관질환의 아형에 따라 망막미세혈관병변과의 상호 연관성은 다를 수 있겠다.
